Alcoholic Beverages & Banquet Permits: (back)

New!!! Online Authorization to Apply for a Banquet Permit form

If you wish to serve alcoholic beverages at your event, you must obtain a Banquet Permit from the Washington State Liquor Control Board.

Here's how it works. Just like the online Request for Use of University Facilities form, this form is filled out online and processed through email. After the applicant pushes "Submit Request," the form is emailed to the Venue Operator (the person in charge of the space you are having your event in) and then to Gus for final approval. After Gus approves, you get an email notifying you that serving alcohol at your event is authorized. You also get a copy of all the terms and conditions you agreed to when you filled out the form, just as a reminder and for your files. Like the UUF system, you are updated by email from eventfrm@u.washington.edu as your form moves through the process.

You take two copies of your authorization email to the Washington State Liquor Store to buy your $10 Washington State Liquor Control Board Banquet Permit. We assume that you keep 1 copy for your files.

Remember, the Authorization to Apply for a Banquet Permit form is only for serving alcohol. If you will be selling alcohol, sponsored by an alcohol producer or distributer, or receiving donations from an alcohol producer or distributor, other forms apply. In those cases, please contact me.

**Special Instructions**

#7a and 7b - Approving Dean/Director/Chair/SAO name (student applicants only) and Approving Dean/Director/Chair/SAO email (student applicants only):

These sections only apply to student applicants. Unless you self-identify as either a student or RSO on #2, these sections will not even be visible. This part of the form is new. If a student is organizing her own party or is organizing an event for an RSO, we now ask for the approval of a Dean, Director, Chair, or SAO representative (Rene Singleton). This is in addition to the logistics approval from the Venue Operator and Gus' final approval. When a student fills out the form for an event, his form will be reviewed by a Dean/Dir/Chair/SAO rep first, then the venue, and finally Gus. RSO events are always approved by Rene Singleton. Other events will need the support of a Dean, Director, or Chair.

#10 - Event Location:

Please choose the location of your event from the drop down. If your event is in an unusual venue or you can't find it in the drop down--such as the parking lot between Balmer and Mackenzie Hall or the Dental School grad student lounge--for example, enter that information in "Other Location." If your event is in "Other Location," you will need to input the name and email of your venue operator or room reservationist. In special situations like this you probably know who your room reservationist is, or, you may contact me to figure it out.

(Some hints: Marie Gary is the special events coordinator for Upper Campus classrooms, Roberta Bustamante of Central Zone Facilities Services is the contact for all outside events on our Seattle campus, Evan Guros is the contact for special events in the Health Sciences Building, and Jo Taylor in Parking is in charge of special event use of our parking lots).

Distinguishing Banquet Permit application emails from UUF emails:

The subject lines of the Banquet Permit application emails start with "==> REQUEST: ", while the UUF emails start with "** UW FACILITIES SCHEDULING." You can sort your email by ==> or ** to distinguish between banquet permit and UUF-related emails. Also, the banquet permit screens have a bright green background, as opposed to the plain white UUF backgrounds. And finally, in the banquet permit email notifications, the first line is a bright orange-red, as opposed to the navy blue of UUF emails.

The Banquet Permit must be displayed in plain sight at your event, and a person 21 years of age must be assigned to serve and confirm that all people drinking alcohol are of legal age and remain in the area at all times.

Note: If UW Catering is bartending your event, your catering contact can obtain the Banquet Permit from the liquor store on your behalf once the UW Authorization form is processed. Or, you may pick up the Banquet Permit yourself directly from the store, but you are already paying for their services so save yourself some time and let them do it.
