Transcripts
Two certified or attested copies of your academic record and the diploma or other document indicating the date and conferral of your degree are required. If the original language of the academic record and diploma is not English, a certified English translation must accompany the original document. A school administrative officer of the institution from which the degree was received must certify official copies of credentials.
Proof of English Language Proficiency
Permanent residents and international applicants are required to take either The Test of English as a Foreign Language (TOEFL) or the Michigan Language Test (MLT) if they do not hold a degree from a U.S. or other English-speaking institution in accepted countries (Australia, Canada, Ireland, New Zealand, or the United Kingdom). It is very important that applicants contact the TMMBA office to ensure that test scores will be valid upon formal admission to the Graduate School prior to applying. Minimum score requirements are listed in Graduate School Memo #8.
Proof of Financial Ability
To be considered for admission, prospective students must submit a Financial Ability Form showing adequate financial resources to cover the cost at least the first year of study. After your financial ability is confirmed, the University of Washington will issue the I-20 or IAP-66. When presented to a US consul, these forms enable a student to obtain either an F-1 or J-1 visa, respectively. Please include the completed financial forms upon submission of your application.
Photocopy of Current Visa
If you are already in the United States, submit a photocopy of your current visa status forms issued to you by U.S. Citizenship and Immigration Services.
Notice of Action
If you are currently in the United States and are submitting a photocopy of your current visa, a signed letter indicating that you will not be requesting an F-1 student visa is required.
